This section covers information collected through altruistichospice.com, including our callback request form. It does not describe how we handle protected health information for patients under our care — see the HIPAA Notice of Privacy Practices below for that.
Information we collect
When you submit our callback request form, we collect the information you provide directly: your name, phone number or email, and insurance provider. We do not use tracking cookies to build advertising profiles, and we do not sell visitor information to third parties.
How we use it
- To respond to your inquiry and discuss whether our services are a fit
- To verify insurance coverage before or during an intake conversation
- To maintain records required by California hospice licensing regulations
Sharing
We do not sell or rent the information submitted through this website. We may share it with insurance payers to verify coverage, or with contracted service providers who help us operate (such as our phone or scheduling systems), under confidentiality obligations consistent with HIPAA.
Your choices

HIPAA Notice of Privacy Practices
As a licensed hospice provider, Altruistic Hospice, Inc. is required by the Health Insurance Portability and Accountability Act (HIPAA) to protect the privacy of patients' protected health information (PHI) and to provide notice of our legal duties and privacy practices.
This notice describes how medical information about a patient may be used and disclosed, and how a patient can access this information. Please review it carefully.
In summary, we may use and disclose PHI for treatment, payment, and healthcare operations without separate written authorization — for example, coordinating a patient's care plan with their physician, or submitting claims to Medicare, Medi-Cal, or a private insurer. Other uses require the patient's or their representative's written authorization, which may be revoked at any time. Patients have the right to request access to their records, request corrections, request an accounting of certain disclosures, and file a complaint with us or with the U.S. Department of Health and Human Services if they believe their privacy rights have been violated, without fear of retaliation.
